Dementia Services

Our dementia service is open to people living in the community who have a confirmed diagnosis of mild to moderate dementia. Over the last few years we have developed specialist expertise and a dedicated team, providing person-centred support for people living with dementia.

How it works

After receiving a referral from an agency, relative or neighbour, one of our skilled and experienced service coordinators will visit the individual at home to assess their needs and capabilities before connecting them to our services and/or signposting them to other local services relevant to their needs.

Befriending for people living with dementia

Living with the symptoms of dementia can make it harder for people to maintain friendships at a time when they are needed most. We offer our befriending service to people with mild to moderate dementia and who are living independently. Our befrienders can provide valuable social support to people living with dementia and help them stay connected to their communities.

If you would like to refer yourself or someone else to our befriending services, you will need to complete a referral form.

Dementia-specific groups

We also have a range of groups specifically for people living with dementia. All of our dementia groups provide a range of mentally, physically and creatively stimulating activities, proven to slow the progression of dementia. We offer escorted transport to the groups allowing many people to attend who otherwise might not be able to.

All of our service users living with dementia are also welcome to attend any other Link Age Southwark group.
